A comprehensive psychiatric assessment that includes a detailed review of your medical, psychiatric, and treatment history, as well as current symptoms, goals, and potential treatment options.
30 Minutes
$175
Established Patient Follow-Up
Ongoing appointments for medication management and supportive psychotherapy. These sessions evaluate treatment progress, address side effects or concerns, and make adjustments as needed.

Computerized Neuropsychological Testing
A computerized ADHD assessment providing objective data on attention, impulsivity, hyperactivity, and emotional regulation.
Note: This test is not covered by insurance and is required for all patients being evaluated or treated for ADHD who wish to establish care with our practice.

Comprehensive ADHD/ADD Diagnostic Package
Designed for adults or children who are already established with another mental health professional or who wish to determine if they have ADHD/ADD.
One computerized neuropsychological assessment

Letter Writing & Documentation
Includes documentation requests for work, school, emotional support animals, and other written letters provided by your clinician.
- Full payment is required prior to letter completion.

Designed for adults or children who are already established with another mental health professional
or who wish to determine if they have ADHD/ADD. Includes:
- One computerized neuropsychological assessment
- Two provider sessions:
- 60‑minute diagnostic evaluation
- 40‑minute feedback session
You will receive a comprehensive evaluation report reviewed directly with you and shared with your
referring provider to guide treatment planning and next steps.
